My Wellness Passport Scale-Up Initiative

What is the initiative?

The My Wellness Passport (MWP) Scale-Up Initiative focuses on expanding the use of YWHO’s data collection platform, My Wellness Passport, and the applicable tools for measurement-based care within mental health and wellness services for youth aged 12 to 25 in Ontario.

Sites that participate in the MWP Scale-Up Initiative are supported with the establishing foundational framework of Integrated Youth Services (IYS) in their area. They are not considered full sites of Youth Wellness Hubs Ontario, and are not included when we refer to our core 27 Hub Networks.

What is the purpose of the Scale-Up Initiative?

This scale-up initiative allows for an expansion of YWHO data collection, which will result in more communities benefiting from measurement-based care (MBC) and integrated service delivery tools and practices. At a smaller scale, this initiative contributes to the evolution and improvement of YWHO services for youth and families. This helps YWHO understand the needs, impacts and successes within the sector.

How did the initiative start?

A competitive process took place in summer 2021 to identify the selected sites for this project, and implementation is now underway, with many sites up and running.
