Announcement of New My Wellness Passport Sites in Ontario: An initiative of Youth Wellness Hubs Ontario (YWHO)

We are proud to launch My Wellness Passport, YWHO’s new data collection platform. 10 health and wellness sites across Ontario will be implementing this platform and apply measurement-based care within health and wellness services for youth aged 12 to 25 years. The 10 new sites are known as My Wellness Passport sites.
